PROGRAM OVERVIEW

Our Employment and Housing program known as Foundational Community Supports (FCS) is a program that helps Individuals who have Medicaid insurance and meet specific medical risk factors including chronic homelessness, complex behavioral health and co-occurring substance use needs as well as a disability or other long-term care needs find and maintain employment and housing by reducing barriers and increasing skills and resources. Supportive Employment specialists teach employment skills, such as identifying job interests, job matching, writing resumes, job searches, interview practice, and support to maintain a new job. Supportive Housing Specialists assist with tools to build and repair credit, assist with application processes, teach skills to be a worthy tenant, and identify financial resources to secure and maintain housing.
